A report to ASA does not replace a report required by law or a report to FDA, FTC, law enforcement, or another authority.
Matters within ASA’s role
ASA publication or record error
Identify the URL, disputed statement, correct information, and supporting source.
Membership or Code concern
Identify the current member, relevant conduct, applicable Code provision if known, dates, and evidence.
Directory or status issue
Identify the member record, inaccurate field, or public use of membership status.
NutraFormula concern
Identify the award record ID, product or formula, recipient, current use, concern, date, and evidence. State whether the matter involves a material formula change, safety or regulatory information, public claim, status, or mark misuse.
Conflict, funding, or editorial concern
Identify the ASA decision, publication, event, reviewer, or relationship and explain the interest that may not have been disclosed or managed.
Conduct in an ASA activity
Identify the event, meeting, platform, people involved, conduct, witnesses, and any immediate steps taken.
Privacy, security, or accessibility issue
Use the dedicated privacy or accessibility route where possible. Do not include passwords, authentication codes, or exploit details in a general form.

Matters handled elsewhere
ASA does not provide emergency response, medical diagnosis, individual legal advice, refunds, product warranties, or binding resolution of ordinary commercial disputes. ASA can determine membership, publication, participation, and program matters within its own authority and may refer information where appropriate.
Information to provide
The form requests:
- concern type;
- name and contact information, or available anonymous option;
- relationship to the matter;
- organization, product, person, program, or ASA record involved;
- URL or record ID;
- description, dates, and location;
- supporting files or primary-source links;
- immediate safety, legal, retaliation, or preservation concern;
- consent and preferred contact method;
- accuracy attestation.
Submit only information you are authorized to provide. Avoid unnecessary health, financial, identity, trade-secret, or third-party personal information.
Anonymous reports
ASA may accept anonymous reports, but inability to ask questions or verify identity can limit review and feedback. Technical, legal, or contextual information may still identify a reporter, so the form does not promise anonymity.
What happens after submission
- Acknowledgment: The system issues a reference without confirming the allegation.
- Triage: ASA checks emergency, mandatory-reporting, jurisdiction, conflicts, preservation, privacy, and immediate public-status needs.
- Assignment: A conflict-screened owner determines the applicable process.
- Information review: ASA may request documents, public sources, or a response from an affected party.
- Decision: The authorized body records findings and action.
- Status and correction: ASA updates a public record when necessary and lawful.
- Closure: The record is retained according to the approved schedule.
ASA may be unable to share details because of privacy, law, fair process, privilege, or an active review. An acknowledgment is not a finding.
Non-retaliation and false reports
ASA rules prohibit retaliation within ASA activity for a good-faith report. A report is not improper merely because it is unsubstantiated. Knowingly false, fabricated, or retaliatory submissions can result in restriction or review.
Confidentiality
ASA limits information to people with a legitimate role but cannot guarantee complete confidentiality. Disclosure may be required by law, necessary for safety or fair process, authorized by the reporter, or needed to correct public status accurately.

Before using the secure form, preserve relevant records and review the scope above. Do not send urgent safety information only to ASA.
